Our cornerstone service. A dedicated case manager helps you identify your needs, build a service plan, and connect with medical, social, educational, and housing resources — then stays with you to make sure it all works.

A case manager completes a comprehensive needs assessment covering health, housing, education, benefits, and family supports.
